Start with the person
A care group brings together the people involved in someone’s support. In Neume, the group centres on a participant’s profile, including their preferences, care information and important contacts.
Bring the right people together
Participants, family members and support workers can be members of the same group. The group owner manages invitations and permissions. Membership does not mean everyone has the same access to every part of the record.
Make the everyday plan visible
Visits, appointments and tasks give people a shared picture of what is coming up. Handovers and messages help connect that plan with what happened during a visit.
Keep it useful
Agree as a group where updates belong and who will keep details current. A shared place works best when the people using it know what to put there and when to check it.
