Check the shared schedule, review the care information available to you and read the latest handover. Preferences can provide useful context: how someone likes to communicate, what they enjoy and what they have planned.
During the visit
Keep the person’s preferences in the conversation. Use your existing care instructions and professional responsibilities to guide the support you provide. Neume organises shared information; it does not direct care or make clinical decisions.
At handover
Use Notes in the participant’s Care Space to find handovers and care updates, with their author and time. Neume includes visit checklists and structured handovers, alongside Chat for conversations. Record outcomes explicitly; elapsed scheduled time does not confirm a visit was completed.
Across your care groups
Independent workers may support more than one person. Neume brings their schedules together while keeping each participant’s information in its own care group.
